Breast augmentation remains one of the most popular cosmetic surgery procedures performed in the United States. Breast augmentation may be performed in several different ways. The most popular method of breast augmentation remains the placement of implants (either silicone or saline). Other methods include fat transfer, which involves the liposuction and injection of the patients own fat to augment the breast. We find this to be more predictable than other methods, such as fat transfer to the breast. Incisions also vary depending on technique and patient anatomy. The most commonly used incisions include the peri-areolar (around the bottom half of the areola), and the inframammary fold incisions. Breast implants may be placed in a sub-glandular position (under the breast tissue, but above the pectoralis major muscle), or a sub-muscular position (under the pectoralis major muscle). Due to decreased risk of capsular contracture and various other reasons, a vast majority of breast augmentations that we perform are sub-muscular (under the pectorals major muscle), with the inferior portion of the implant under the breast gland - making it a “dual plane” breast augmentation (under 2 planes). The decision regarding silicone vs saline depends on the patients anatomy, incisional preferences, patient age (the minimum age requirement for silicone breast augmentation is 22), and amount of overlying breast tissue.
